Eyewear and Insurance Policies
Woolf Eye Lab should be made aware of all current insurances in your possession. When applying insurance, all copays MUST BE PAID at time of service. Insurance will be submitted on date of service and cannot be submitted afterward. Any upgrades to frames, lenses, and lens options will be the patient’s responsibility.
All frames have a ONE YEAR WARRANTY against MANUFACTURERS DEFECTS ONLY. Warranties DO NOT COVER chewing, accidents, thefts, loss, etc. Glued frames and lenses VOID manufacturers warranties. MOST LENSES are covered under a 1 YEAR SCRATCH WARRANTY and WEL will honor ONE doctor’s change within 45 days of receipt from the lab at no-charge within the same lens style. Any changes after are subject to cost.

Refunds
Should you receive an order and find it unsatisfying, we will work with you to identify a better solution. Due to the nature of medical devices, order cancellations are subject to lab processing time and fees. We can no longer accept contact lens returns or unopened or opened boxes. We will be more than happy to remedy the situation or order to the best of our ability.
